About Alden Terrace
Alden Terrace provides skilled nursing and supportive care through an interdisciplinary, team-based approach focused on each resident’s individualized plan of care. Services are delivered in a structured, supervised environment designed to support residents’ medical, functional, and daily care needs.
Our care teams work collaboratively with residents, families, and physicians to coordinate services in alignment with physician orders and established care plans. Family members and loved ones are welcomed as part of the care process and encouraged to participate in care discussions as appropriate.
Our nursing team includes Registered Nurses (RNs), Licensed Vocational Nurses (LVNs), Certified Nursing Assistants (CNAs), and Restorative Nursing Aides who work together to provide ongoing nursing care, monitoring, and assistance based on individual needs.

Skilled Nursing Care
Our skilled nursing team provides 24/7 clinical care with professionalism and compassion. From managing complex medical conditions to assisting with daily needs. We provide nursing care in accordance with physician orders and individualized plans of care to support residents’ daily medical and personal needs.
Rehabilitation Services
Our rehabilitation services support residents through structured therapy programs focused on mobility, function, and daily living skills. Services may include physical, occupational, and speech therapy, with care plans tailored to individual needs and goals as determined by the care team.
Dementia Care Services
Our secured memory care unit provides a safe, structured environment for residents with dementia or wandering tendencies. With 24/7 locked exits, daily enrichment activities, and continuous skilled nursing, we provide specialized support with a focus on resident safety, supervision, and structured daily routines.
Ancillary & Specialty
Services
Residents have access to a wide range of on-site specialty services, including optometry, dentistry, dermatology, audiology, psychology, and wound consultation. These services support whole-person care without the need to leave our facility.
Life Enrichment & Activities
Our Life Enrichment program offers daily activities that promote social interaction, creativity, and joy. Activities are offered to encourage social interaction and participation based on resident preferences and abilities.
